The Permanent Forum recommends that States, the World Health Organization (WHO), the United Nations Population Fund (UNFPA), the United Nations Development Programme (UNDP) and other relevant organizations adopt targeted policies, programmes, projects and budgets designed to address the staggering prevalence of diabetes among indigenous peoples and put in place culturally appropriate health services, health education and awareness-raising initiatives to treat diabetes and prevent its rapid growth. Particular attention should be given to pregnant women, whose reproductive health is closely linked to the future risk of their children developing diabetes.

The Forum recommends that States, United Nations agencies, bodies and funds, other multilateral bodies and financial institutions and other donors provide technical and financial support to protect and nurture indigenous peoples’ natural resource management, environment-friendly technologies, biodiversity and cultural diversity and low-carbon, traditional livelihoods (pastoralism; rotational or swidden agriculture; hunting and gathering and trapping; marine and coastal livelihoods; high mountain agriculture; etc.). The Forum further recommends that discussions and negotiations on strengthening the links between climate change, biodiversity and cultural diversity under the Convention on Biological Diversity or the United Nations Framework Convention on Climate Change ensure the effective participation of indigenous peoples.

Recalling paragraph 40 of its report on its sixteenth session (E/2017/43-E/C.19/2017/11), the Permanent Forum calls on Governments in the Arctic, Eastern Europe, the Russian Federation, Central Asia and Transcaucasia, along with academics, to take appropriate measures to introduce the endangered languages of their regions into educational practices and include the learning of those languages in curricula at all levels of educational system, when requested by indigenous people.

The Permanent Forum reiterates previous recommendations that WHO, the United Nations Children’s Fund (UNICEF) and UNFPA, as well as regional health organizations and Governments, fully incorporate a cultural perspective into health policies and programmes and reproductive health services aimed at providing indigenous women with quality health care, including emergency obstetric care, voluntary family planning and skilled attendance at birth. The roles of traditional midwives should be re-evaluated and expanded so that they may assist indigenous women during their reproductive health processes and act as cultural brokers between health systems and indigenous peoples.

The Permanent Forum recommends following the example of indigenous peoples, who have been the stewards of the land and sea for millenniums. When allocating research and development funding and setting the criteria for clean development mechanism projects, policymakers at the State and multilateral levels must look beyond the simple question of whether a particular form of alternative energy or carbon absorption technique can provide a short-term reduction in greenhouse gases. Policymakers should consider the long-term sustainability of any mitigation policy they choose.
